ONLINE SPORTS REGISTRATION

We are going to a new online sports registration site for the 2017-18 school year as so many parents were having trouble registering this year.  In order to play sports at Lake City High School you will need to register your student at registermyathlete.com/schools.  BEFORE  tryouts/practices start.  You may register from your phone if you don’t have access to a computer.

Registration Instructions: 

1. Find your school by going to http://registermyathlete.com/schools/, select State (Idaho) and find your school.  Click Lake City High School to continue to the next step. 
2. Parents need to create an account by clicking on the “Create an Account” button.  The system will show 2016/17 school year until June 1^st so please put current grade. 
3. Add a New athlete – Click on Add Athlete on left-hand side of the page.  This only needs to be done once during your athlete’s entire career at a school as it carries over year to year.  It includes emergency contacts and medical information
4. Register for a Sport – Click “+Register for a sport” to begin registration, you will be asked to choose which sport your athlete is registering for.

Forms needed to play and tryout:

Physical:  (needed 9^th and 11^th grade year) must be completed after May 1^st of your 8^th grade year or you will need a new one.  Physicals are good for 2 years.  Parent must fill out first page and sign.  Doctor fills out second page.  You will upload the physical, once completed.  If you don’t have a scanner you can take a picture of it with your phone and upload.   If you physical is still current and on file at the school just do the interim questionnaire and I will upload the physical. 

Interim Questionnaire:  Needed on the years you don’t turn in a physical (usually your 10^th and 12^th grade, 

Activity Contract:  Click on electronic documents and then read document to view the athletic contract.  You will need to check the box I Agree and then guardian and student will both need to E-sign. 

Health Insurance:  When registering your student you will need to fill out health insurance information.  If you do not have health insurance you will need to purchase school insurance.   See the activities office for applications. 

ASB fee:  All students must pay the $40 ASB fee to play sports.  You may pay it in skyward or at the school.

Travel Fee:  All students must pay $25 travel fee unless you have an approved free or reduced lunch application on file at the school.  This must be done each school year in order to be waived.  You need to pay the fee at the school and fill out the form specifying which activities you are paying.  There is a $75 limit per student or $150 per family.  A travel fee does not need to be paid until after your student makes the team. 

Voluntary Drug Enrollment Form:  Can be found online and is strictly voluntary.  

All schedules will be on lakecitysports.com.  Click on the season and pick what sport.    All cancelations or changes will show up there immediately.  Team pictures will also be posted there.
